Start with a name and a licence type
Maricopa County lists Surprise among its communities.
A licence is the state’s permission to work in a named care role.
Ask for the full name and kind of licence of the person giving care. Search that person’s record, rather than the name of the office.
For doctors with MD after their name, the Arizona Medical Board has a licence search and links to the board’s written decisions about doctors.
Look for those decisions as well as the licence. Ask the board to explain any decision you do not understand.
A doctor using DO after their name has a separate Arizona board with its own licence search.
Arizona’s Nursing Board links to searches for a nurse’s licence and any certificate for a care role that needs extra training.
A nurse practitioner is a nurse trained to assess health problems and choose care.
Under the October 3, 2026 rule, a nurse practitioner must have permission from the Nursing Board to prescribe. The medicine must fit the nurse’s training.
Ask the board to check the nurse’s licence, certificate and permission to prescribe.
Physician assistants, or PAs, have an Arizona board with its own licence search.

For online care, say where you will be
Tell the service your physical location for the visit, especially when travel is planned.
As checked October 3, 2026, Arizona board registration lets some care professionals with other states’ licences give online care to people here.
That registration is permission for remote care under the October 3, 2026 rule; it is not a full Arizona licence and does not permit an office visit here.
Ask which Arizona board records their permission to treat you, and for the licence or registration number. Ask that board to confirm whether it covers your visit.
If they say a different rule permits the visit, ask them to name the rule and ask the board to explain it.

Check the pharmacy as a separate step
Ask which pharmacy will prepare and send the medicine. Write down its full name before checking the record.
Under the October 3, 2026 rule, a pharmacy sending medicine here from another state needs an Arizona permit. It also needs a current permit or equivalent permission in its home state.
Ask the pharmacy for both permit numbers and the names of the boards that issued them.
The U.S. Food and Drug Administration (FDA) is an agency that reviews medicines.
The FDA’s pharmacy-check page points readers to each state’s licence records.

Read a record with its limits in mind
As checked October 6, 2026, Arizona sets limits on online licence records and allows requests for extra public records.
If the online record lists no penalties or other board decisions, that does not prove there were no past complaints or decisions.
